7. I was born October 31, 1962 in Kentucky, USA and am 45 years of age. My citizenship and allegiance [by birth] is American. My last foreign domicile was in Ohio, USA during 1993-97;

8. My personal/physical description is male, brown hair, blue eyes, fair complexion, 180 cm height and 80kg weight. I speak, read and write English and know basic Spanish/ Tagalog;

9. I express the desire to possibly change or modify my name during the naturalization process to facilitate integration and assimilation.
