Mr., Mrs., or Miss (last name) has requested a hearing which will be held in the near future. You are invited to attend the hearing to testify before the Board and the employee and his counsel, if any, and be subject to cross-examination. If you signify a willingness to appear, you will be advised of the time and place of the hearing which will be held in (city and state). (If a hearing date has been set, indicate time and date.) If you do not care to testify before the employee, you may appear in private before the Board and your name will be held in confidence.
