Whereas, the Petitioner alleges that a Decree No. 764148, issued by the Court of the First Instance, Province of Laguna, dated December 23, 1941, for the purpose of Land Registration but no Original Certificate of Title has been issued on parcel of land described as follows:

"A parcel of land (of Lot 4518 of the Cadastral Survey of Pagsanjan Cadastre). Situated in Barangay of Pinagsanjan, Municipality of Pagsanjan, Province of Laguna, Island of Luzon. Bounded on the N., along line 3-4 by Lot 4519; on the N.E., along lines 4-5 by Lot 2246; on the N.E., and S.E., along lines 5-6-7-8-9-10-11-12 by Lot 2245; along line 12-13 by Lot 2244 all of Cad. 69, Pagsanjan Cadastre; on the S.E., and S.W., along lines 13-14-15 by Creek; and on the W., along lines 18-1-2-3- by Cad. 69, Pagsanjan Cadastre. Beginning at a point marked "1" on plan being N. 68 deg. 27'E., 796.54 m. from BBM. No. 2, Cad. 69, Pagsanjan Cadastre."

Wherefore, you are hereby given notice that the petition is set for hearing on December 4, 2013 at 2:00 o'clock in the afternoon, before Branch 91 of the Court, Santa Cruz, Laguna, on which date, time and place, you should appear to file your claim or objection, if you have any to the petition and have this notice be published in Official Gazette.

A Verified Petition dated January 10, 2013 has been filed with this Court under the provisions of R.A. 26 by petitioner Ernesto Cariaso for the reconstitution of the lost original copy of Original Certificate of Title No. 0-2293 alleged to have been destroyed by fire last August 26, 2000 as per Certification issued by the Register of Deeds and the owner's duplicate copy alleged to have been lost/missing, praying the Honorable Court that after due publication and notice, an order be issued directing the Register of Deeds of La Union to reconstitute in favor of the registered owner the Original Certificate of Title No. O-2293, and to issue a New Owner's Duplicate Copy with the same legal effect as that of the lost one.

Now therefore, it is hereby ordered that the Notice of Initial Hearing be published at

the expense of the Petitioner, twice in successive issues of the Official Gazette, and to be posted at the Bulletin Boards of the Municipal Hall of Bacnotan, La Union, Bulletin Board of the Provincial Capitol of San Fernando City, La Union, Bulletin Board of the Justice Hall of San Fernando City, La Union and at the Barangay Hall of Barangay Casiaman, Bacnotan, La Union, where the property is located at least thirty (30) days prior to the date of Initial hearing on January 15, 2014 at 1:30 o'clock in the afternoon.

Let copy of this Notice of Initial Hearing be sent by registered mail at the expense of the Petitioner to the person or persons who are now actually possessing the property, the owners of the adjoining properties, the Registrar, Register of Deeds of La Union, The Commissioner, Land Registration Authority, the Director, DPWH District Office of La Union and all persons who may have interest in the property, to appear and file their claims or objections to the petition.

This pertains to the Verified Petition filed by Tiong Gui Chapel, Inc., represented by entries in Transfer Certificates of Title No. GO TIC CHING seeking for the correction of T-12085 and T-1230 particularly the name of the registered owner thereof from "TIONG GUI CHAPEL" to its "TIONG GUI CHAPEL, INC." and the cancellation or deletion of the names of petitioner's representative appearing on the said certificates of title by the Office of the Registry of Deeds of Tacloban City.

Finding the Petition to be in due form and substance, the same is given due

course.

Let this Order be published at the expense of the petitioner twice in the Official Gazette and be posted at the Bulletin Boards of the Sangguniang Panlungsod of Tacloban City, Sangguniang Panlalawigan of Leyte, Barangay Hall of the place where the property is situated, Registry of Deeds of Tacloban City and in the Bulletin Board intended for this Court at the Bulwagan ng Katarungan in accordance with existing rules and procedure at the expense of the petitioner.

The property was declared for tax purposes which were fully paid for the full year of 2013. The adjoining owners of the property are: Chua Shiao Cheng Lim, William Chua married to Paulina Uy Chua, Wilson Uy Chua married to Lily C. Chua, Wilfred Lim Chua married to Josie Chua, Winnie Chua Go married to Frederick Dy Go and William Chua Chan married to Benedict T. Chan all at Lot 18 Block 5, Sunflower Cypress Village, Quezon City; Teresita Tanbonliong married to Carlos Tanbonliong of No. 27 Kingsville Street, White Plains, Quezon City; Sander Uy Sy married to Catalina Ong Sy of No. 106 Dapitan Street, Sta. Mesa Heights, Quezon City. Petitioner also alleges that no co-owner's or other duplicates of said titles have been issued and no deed or other instrument affecting the property embrace in and covered by TCT No. N-163326 has been presented or is pending registration in the Registry of Deeds. Lastly, there is no improvement or building in the said property as evidence by a Certification dated July 26, 2012 issued by the City Assessor's Office of Quezon City.

Finding the petition to be sufficient in form, the same is hereby set for hearing on December 9, 2013 at 8:30 o'clock in the morning, before this Court, located at the Basement, Hall of Justice, Quezon City Hall Compound, Quezon City, on which day, time and place, all interested parties are hereby ordered to appear and show cause why the petition should not be granted.

Let copies of this Order, together with a copy of the petition and its annexes, be furnished the Office of the Solicitor General, the Administrator of the Land Registration Authority, the Executive Regional Director of the National Capital Regional Office of the Department of Environment and Natural Resources, Visayas Avenue, Quezon City, the Register of Deeds of Quezon City, the

Let this Order be published in the Official Gazette once a week for two (2) consecutive weeks. Also, let copies of this Order be posted by the Branch Deputy Sheriff/ Process Server at the main entrance of the Quezon City Hall, and in the bulletin boards of the Sheriff's Office of the Regional Trial Court of Quezon City and of this branch of Court as well as in the barangay hall of the barangay where the subject land is situated, at least thirty (30) days before the scheduled date of hearing, all at the expense of the petitioner.

Petitioner is hereby directed to serve copies of this Order on all the adjoining owners immediately upon receipt of a copy of this Order, and to make the necessary returns therefor.
